About This Service

Kyabra’s Supported Accommodation Service can offer transitional housing and case management support for individuals and families who are experiencing homelessness, or are at risk of homelessness.

The Supported Accommodation Service provides families with transitional housing through rolling three-month lease agreements. During this time, a case worker partners with community members to develop an individualised case plan and deliver wrap around supports to address barriers to accessing and maintaining stable housing. The program also supports community members to build the skills necessary to sustain long-term accommodation.

Our Sunshine Coast site may be able to offer temporary accommodation at times when there are vacancies.

Support may include:

  • Short to medium-term transitional supported accommodation
  • Individualised case planning to identify goals in relation to longer-term housing pathways
  • Support to overcome barriers to access housing  and sustain long-term accommodation.
  • Support to explore all available housing options
  • Referrals to specialist and community services to meet identified needs
  • Outreach case management
  • Most referrals for our supported accommodation are made through the Queensland Homelessness Information Platform (QHIP). QHIP referrals are only possible when vacancies are listed on the platform.
